About this data

The information set out in this scorecard presents an overview of the health and wellbeing outcomes for people in each local area. We have tried to give a sense of the health of people at different stages of the life-course, which provides the context within which to understand how well services are doing in improving the health and wellbeing of local people in the places they live.

All indicators we have selected are already published as part of the Public Health Outcomes Framework (PHOF), the Adult Social Care Outcomes Framework, or the Clinical Commissioning Group Outcomes Indicator Set, to ensure greater consistency and alignment with established improvement activity.
