I want to change my personal details held by the NHS

Personal details like your address and contact telephone number are held by the individual NHS Services you are registered with e.g. GP, Dentist, Optician etc.

To change this information you will need to contact the services in turn and ask them to update their records.

I want to reset my password

You should receive an email asking you to reset your password, please check your junk email folder in case the email is there. If you do not receive an email to reset your password, please let us know here.

For medical professionals:
Please note that your username and passwords are the same to edit your profile and to respond to comments. You would need to follow the step above if you need a password reset for either system.
