Norfolk Community Health and Care NHS Trust (NCH&C) is an important part of daily life for many people right across Norfolk. We serve a population of 870,000 people in and around Norfolk, making us one of the largest providers of community health and care services in the NHS.
Our expert staff are working hard all over the county, making sure you receive the right care when you need it, close to where you live and work. Our specialist teams all across Norfolk provide personalised health and care services for everyone, from babies to the elderly.
Specialised care for all
We help to keep people healthy with our Stop Smoking service and work with patients to regain their independence and quality of life after a brain injury, stroke or fall. We also provide end-of-life care services both at people's homes and in specialist community units, to allow people to pass away comfortably and with dignity.
We care for the most disadvantaged and vulnerable in our communities who can find it very difficult or daunting to get the right healthcare, for example, migrant populations, sex workers or homeless people. We also support people of all ages with Learning Disabilities, right across the county - empowering them to live healthy and independent lives.

We listen to our patients and use their feedback to improve our services. Our most recent patient experience scores (June 2015) show that 99 per cent of our patients would recommend us to family and friends.
