Who we are

Liverpool Community Health NHS Trust is at the heart of the delivery of healthcare within the communities of Liverpool and Sefton reaching a population of approximately 750,000 people.

We have a team of over 3000 staff – 80 percent of whom are practising health professionals including Nurses, Community Matrons, Health Visitors, GPs, Dentists, Dieticians, Podiatrists, Physiotherapists, Occupational Therapists and Speech and Language Therapists.

Our staff and services come into contact with hundreds of thousands of people in our local communities every year and therefore are often involved not just with patients, but also their families, carers and the communities in which they live. This means our staff can help to understand patients’ holistic health and social needs, including education, prevention, early detection and self-care.

 What we do

Liverpool Community Health NHS Trust deliver community health services to people in their own homes, in over 100 locations including health centres and clinics, 5 Walk-in Centres, Broad Green Wards 9 to 11 and Aintree Hospital Ward 35 – Intermediate Care Centres. We also deliver specialist Offender Health Care, Medicines Management and Nutrition and Dietetic Service

Our services enable people to be cared for in their own community and remain as independent as possible, rather than having to go to hospital.  Many of these services are delivered out-of-hours, with 24 hour, 7 day a week access.

Community Foundation Trust

In 2008 we were selected as one of eight pilot NHS organisations by the Department of Health to work towards the proposed model of Community Foundation Trust status. This programme aims to significantly improve community health services, both nationally and within our communities.
